(2) HSC Mark Sheet
(3) Graduation Certificate
(4) Caste Certificate issued by the Competent Authority.
(5) Discharge certificate in the case of Ex-Servicemen
(6) Certificate  issued  by Appropriate Authority  in  appropriate  proforma,  showing  the
percentage of disability, in case of Persons with disability (PWD).
(7) Self-addressed envelope of size 23x12 cm, duly affixed with postage stamp of value
Rs. 5.00.
(8) Two recent recognizable Passport size photographs - one copy to be affixed on the
application form & duly signed across and another for Exam hall ticket purpose &
(9) Self-addressed post card, if acknowledgment is required.
